1. Do you have a first-degree (mother, sister or daughter) relative with breast cancer?
2. Do you have a relative diagnosed with both breast and ovarian cancer?
3. Do you have a relative diagnosed with breast cancer in both breasts?
4. Do you have several relatives in the same lineage diagnosed with breast cancer?
5. Do you have several relatives diagnosed with breast cancer at young ages, especially before onset of menopause?
6. Do you have any men in your family diagnosed with breast cancer?
7. Have you ever had a breast biopsy and been told the cells were atypical?
8. Have any of your family members ever been told they are positive for the breast cancer gene?
