<p><strong>WORCESTER</strong> - UMass Memorial Home Health was recently named to the 2009 HomeCare Elite, a compilation of the most successful Medicare-certified home health agencies in the nation by Outcome Concept Systems (OCS) and DecisionHealth. UMass Memorial ranked in the top 25 percent of home health care agencies in the country and was cited for its outstanding performance in quality patient care outcomes and improvement. UMass Memorial Home Health was the only home health provider in Worcester to earn this honor.</p>
<p>Fully accredited by the Joint Commission and certified by the Massachusetts Department of Public Health, UMass Memorial Home Health supports patients in remaining independent at home. The UMass Memorial Home Health staff members provide patients with quality health care assistance, including skilled nursing, therapy and health education programs that help prevent future hospitalizations and offer patients an opportunity for early hospital discharge. Home Health staff members develop individual care and treatment plans by assessing each patient's needs in collaboration with the patient and his or her family and physician.</p>
