Diabetes Collaborative Project

The Diabetes Collaborative Project is designed to improve the management of patients with diabetes in the primary care setting. It builds on current resources and strengths within UMass Memorial Health Care to deliver care using the planned care model of chronic diseases which is evidence based, guideline driven and includes an informed care team working with actively engaged patients. Participating practices have support to:

  • Improve office processes
  • Offer diabetes group visits and on-site diabetes education
  • Generate data to monitor and sustain improvement gains
